What form do I fill out for my Queensland trainee user laser licence?

If we had a dollar for every time we've been asked this we'd be giving our courses away for free we'd be so rich LOL! This trainee licence is the one you get first and then you do your supervised hours and competence assessment to get your full one later. Think of it like being on "training wheels".

You start with your trainees licence and then progress to your full one.

1. TO GET YOUR TRAINEE LICENCE:

First you start on a Trainee licence so submit Form 3 Application for a license to use laser apparatus - Cosmetic Purposes (Trainee).

To do this you will need to provide:

  1. A certified copy of your qualifications (e.g. AHPRA registration, Diploma of Beauty Therapy)

  2. A certified copy of your laser safety training certificate or evidence of enrolment, as appropriate. This is your Laser & intense pulsed light safety training course (cosmetic practice) from Bravura Education. We are numbers: 7, 8 & 9 in the dropdown box on the application form depending on which course you've done.

  3. A certified copy of your evidence of completion of an infection control training module. If you are a registered health practitioner this is covered, otherwise you will have to show evidence that you have completed an acceptable course(s). This is generally covered in Beauty Therapy qualification but if you do not have this radiation Health Queensland has a list of approved courses. Radiation_Health@health.qld.gov.au

  4. Certified copies of each of your proof of identity documents

  5. Payment of application fee.

After you have posted or faxed the Application for a license to use laser apparatus - Cosmetic Purposes (Trainee) with your evidence and received your Trainee license and accompanying information it will be clear to you what your license category is, the scope within which you are able to practice and the number of hours that will be required before being eligible for a full license.

Category 1 = hair removal TOTAL = 25hrs

Category 2 = hair removal = 25hrs, skin rejuvenation = 50hrs, superficial vascular lesions = 50hrs, superficial pigmented lesions = 75hrs TOTAL= 200hrs

Category 3 = hair removal = 25hrs, skin rejuvenation = 50hrs, superficial vascular lesions = 50hrs, superficial pigmented lesions = 75hrs, tattoo removal =1 00hrs TOTAL =300 hours.

Then you then need to do the following:

  1. Only carry out the radiation practices listed on your license and abide by any conditions listed

  2. Identify and have an arrangement with a person who holds an unrestricted Queensland license for the procedures you would like to be deemed competent at and ask them to be your supervisor/mentor. Can't find one? Consider spending time with our trainer in Lismore, Northern NSW to gain practical experience, log hours and gain confidence.

  3. Maintain a log book of all the procedures you perform that is signed by your supervisor mentor

  4. Complete the allocated hours associated with the relevant category for each procedure (As above: Category 1=25hrs, Category 2=200hrs, Category 3=300 hours)

When you have completed your log book your supervisor/mentor then signs you off with a Trainee Cosmetic Laser Operator – Statement of Competency which you submit to: Radiation_Health@health.qld.gov.au Then you are ready to apply for a full licence.

2. TO TRANSITION FROM TRAINEE TO FULL USER LICENSE:

Here's the link you need. You will need a certified copy of your Laser Safety Certificate, identification with a photograph, infection control qualification (if you are AHPRA registered that's sufficient) and to pay the licensing fee.

Here's some hints to make it easier:

  1. SECTION B: DETAILS OF QUALIFICATIONS AND TRAINING - Write: Bravura Education - Laser Safety Officer Certificate

  2. SECTION D: TYPE OF RADIATION SOURCE YOU ARE APPLYING TO USE - Tick: Laser apparatus box

  3. Q1- SECTION F: DETAILS OF RADIATION SOURCES PROPOSED TO BE USED - Write: Non applicable (you are not using ionising radiation)

  4. Q2- SECTION F: DETAILS OF RADIATION SOURCES PROPOSED TO BE USED - Apparatus Category = C.1 Laser apparatus; Purpose category = hair removal, tattoo removal etc

  5. EQUIPMENT TYPE - Cosmetic laser 

  6. Be patient, the process to receive your licence can take a little while. We suggest you complete your course and lodge the paperwork as quickly as you can to shorten delays.
